Sample Entry Form:	Sit back and relax. Members pick their team of 10 players and compete against fellow members for gift certificates to be utilized throughout the club! At the end of each day we will tally the scores based on the 4 best scores from the team of 10. Each Participant will receive an email update at the end of each day's round as to where they rank against fellow members.
